Membership Criteria
- You must be a Registered or Licensed nurse in your own country
- You must work in the specialty of diabetes (clinical practice, education or research)
- Employees of the pharmaceutical industry are not eligible

This is available in two versions:
Excel file (.xls) which you can fill in and return by email (preferred option)
Acrobat PDF file (.pdf) e.g. if your
computer does not have the Excel programme. (This version can preferably be filled in on
your computer but can also be printed, filled in by hand
and returned by post)
